Choice is really important at the end of life. Many people want to be cared for in their own home surrounded by those people who are important to them and our Hospice Care at Home service makes that possible by providing care and support.
All people assessed and accepted on to the service are supported with a maximum of four care visits per day by two Home Care Assistants and based on individual need. The Fast Track service is for people who:
- Have been assessed by a healthcare professional to be in their last weeks or days of life
- Have a rapidly deteriorating condition
- Live in the Furness and South Lakes area
- Are over the age of 18
- Have chosen to die at home
Care and support is delivered through this 12 week service between the hours of 7.30am – 10.30pm seven days a week.
Once a referral is received, the Hospice Care at Home co-ordinator organises an initial assessment by an Integrated Team sister – this provides a baseline of where the patient is conditionally. Following assessment will take place at week 4 and week 7. At week 7 this meeting is followed up by a multi-disciplinary team meeting to ascertain if the patient still meets the criteria for the service.
