Hospice at Home Referral Process
If you are a Health Care Professional and you would like to make a referral please use the following steps:
- Complete the referral form (available in the downloads column) with as much detail as possible.
- Fax the completed referral form to St Mary's: 01229 583072.
- Ensure that you include your contact telephone number as we will need to inform you of the outcome of the referral.
For the Overnight Service:
- Follow the same procedure as above.
- Same day requests must reach us before 3pm. We will do our best to provide an overnight carer the same night but this cannot always be guaranteed.
- We will review each patient regularly and always after three nights of care.
